Introduction:

  • Common root cause analysis methodologies: 5 Whys, Fishbone diagram, Fault Tree Analysis, etc.
  • Applying root cause analysis techniques to different incident scenarios
  • Facilitating root cause analysis meetings and discussions

Day 4: Developing Corrective and Preventive Actions

  • Defining corrective and preventive actions (CAPAs) and their role in incident management
  • Linking root causes to effective CAPAs for preventing recurrence
  • Designing practical and feasible action plans to address identified issues
  • Monitoring and tracking the implementation of CAPAs
  • Case studies and exercises on developing effective CAPAs

Day 5: Incident Investigation Report and Improvement Strategies

  • Structure and content of an incident investigation report
  • Writing clear and concise investigation reports that meet ISO 45001 requirements
  • Communicating investigation findings to stakeholders and management
  • Incorporating lessons learned into the safety management system
  • Final workshop and action planning: applying learning to real-world incident investigation scenarios
